a)                     AGREEMENT - Request approval and authorization for the Executive Director to sign the Modification #2 to the subcontract agreement (Misc. Agreement #2023-153) with Advocates for Human Potential, Inc. (AHP) for the purpose of providing funding for the increase, improvement, and expansion of the California Hub and Spoke System, thereby increasing the total funding amount to $162,084.81, and extending the period of performance to 1/1/23 through 9/29/24

body

Financial Impact:

This is a receivable to the Tehama County Health Services Agency (TCHSA) Medical Clinic in the amount of $162,084.81. The funding for the subcontract has been increased by $35,059.81, bringing the total from $127,025.00 to $162,084.81.

Background Information:

The subcontract agreement between AHP and the County for the Hub & Spoke Project was approved by the board on 6/6/23 and expired on 6/30/24. This amendment will extend the Period of Performance to 9/29/24 and increase the funding amount to $162,084.81.

The California Hub and Spoke System (H&SS) of the California Medication Assisted Treatment (MAT) Expansion Project has been eliminated. All funding from this project was absorbed by the Department of Health Care Services (DHCS) and redistributed into a new program titled "State Opioid Response (SOR) 3". DHCS chose Advocates for Human Potential (AHP) as the funding entity for this program.

We are seeking approval of the subcontract Modification #2 for the Hub and Spoke Project in an effort to improve access to services for Opioid Use Disorder and continuation of the California DHCS Hub and Spoke System. Funding will support the purchases of telehealth equipment to assist with access to healthcare services at the Medical Clinic.
