Financial Impact:
If approved, the requested action would result in an annual increase of approximately $157,552.06, which includes salary and benefits at Step E. These expenses will initially be funded by the Providing Access Through Health (PATH) round three grant. Continued funding will be provided by revenue generated via Medi-Cal fee for service billing. There is no impact to the General Fund.
Background Information:
The Sheriff’s Office and the Probation Department are requesting to add a new classification for a Cal-AIM Program Coordinator.
California’s Department of Health Care Services (DHCS) launched the Cal-AIM (California Advancing and Innovating Medi-Cal) Justice-Involved Reentry Initiative to improve care coordination and access to health care services for individuals leaving incarceration. This initiative mandates the establishment of structured systems to support care transition, enhance continuity of services, and ensure proper billing and data reporting for Medi-Cal-eligible justice-involved individuals.
If approved, the Cal-AIM Program Coordinator will be shared between the Sheriff’s Office and the Probation Department and will oversee all aspects of the Justice-Involved Initiative requirements for the Jail and the Juvenile Detention Facility. Responsibilities will include organizing implementation efforts, maintaining communication with partner agencies and providers, monitoring compliance requirements, developing internal procedures, and ensuring continuity of care planning for eligible individuals prior to release. Additional responsibilities will include the supervision of pre-release care functions as well as oversight of billing reimbursement. To effectively implement this program, many counties are in the process of developing similar classifications.
Based on the minimum qualifications, essential functions, and scope of responsibility associated with this classification, the proposed salary placement is Range 51. This placement aligns with other County classifications with similar minimum qualifications and responsibilities.
The Joint Council bargaining unit has been notified of the requested actions and an agreement has been reached.
