Background Information:
The Tehama County Health Services Agency (TCHSA) is requesting the approval of three (3) new classification specifications for Institutional Licensed Crisis Health Worker I/II, Institutional/Crisis Health Registered Nurse I/II and Supervising Institutional/Crisis Health Registered Nurse.
The creation of these positions stems from a mandate issued by the California Department of Health Care Services (DHCS) requiring changes to crisis services. As part of the American Rescue Plan Act (ARPA), mobile crisis services were added as a new benefit in the Medi-Cal Program. Beginning January 1, 2024, all counties in California, including Tehama County, are required to have a 24/7 mobile crisis response team integrated into the community, per Behavioral Health Information Notice 23-025 (BHIN 23-025). In order to comply with this mandate, TCHSA has stopped crisis stabilization services delivered in a brick and mortar facility, which was referred to as the Tehama County Mental Health Community Crisis Response Unit (CCRU), and has transitioned to mobile crisis services delivered in community-based settings.
With this transition, and in order to comply with the mandate, there is a need for new classification specifications to reflect the updated job duties being performed. The new classifications reflect changes that are associated with the assignment of duties relating primarily to the field services of the Mobile Crisis Response Unit of the Health Services Agency. If approved, these job classifications will replace the Institutional Licensed Vocational Nurse (LVN)/Licensed Psychiatric Technician (LPT) I/II, Institutional Registered Nurse (RN) I/II, and Supervising Institutional Registered Nurse classifications. There is no requested change to the salary range placements.
In addition, the Department requests approval to amend the FY 2023-24 Personnel Allocation List (PAL), (Reso #2023-88), by deleting four (4.00 FTE) Institutional Licensed Vocational Nurse I/II Bilingual, Institutional Licensed Psychiatric Technician I/II Bilingual, or Institutional Registered Nurse I/II Bilingual allocations, deleting three (3.00 FTE) Institutional Licensed Vocational Nurse I/II, Institutional Licensed Psychiatric Technician I/II, or Institutional Registered Nurse I/II allocations, deleting one (1.00 FTE) Licensed Clinical Supervisor allocation, adding six (6.00 FTE) Institutional Licensed Crisis Health Worker I/II Bilingual or Institutional/Crisis Health Registered Nurse I/II Bilingual allocations, adding three (3.00 FTE) Institutional Licensed Crisis Health Worker I/II or Institutional/Crisis Health Registered Nurse I/II allocations, and adding one (1.00 FTE) Supervising Institutional/Crisis Health Registered Nurse Bilingual allocation, effective 3/5/2024
The Department currently has five (5) incumbents in the Institutional LVN II classification and one (1) incumbent in the Institutional LPT II classification. If approved these incumbents will be reclassified to an Institutional Licensed Crisis Health Worker II. Additionally, there is one (1) incumbent in the Supervising Institutional RN classification, and if approved, will be reclassified to a Supervising Institutional/Crisis Health Registered Nurse. The Department’s existing Licensed Clinical Nurse Supervisor allocation, which is currently vacant, will be reclassified to a Supervising Institutional/Crisis Health Registered Nurse.
If approved, this will result in a salary range increase from Range 35 to Range 39 for the incumbent currently in the Institutional Licensed Psychiatric Technician II position, as the new position’s salary range will be aligned with the Institutional LVN classification. Although these classifications are shared with the jail nursing unit, there will be no additional cost for the jail nursing unit, therefore, no County general fund cost will be incurred.
The Joint Council Bargaining Unit has been notified of the proposed modifications and are in agreement with the changes.
