Background Information:

This is a new agreement with SacValley Medshare, which is a Qualified Health Information Organization in California, to facilitate and operate electronic health information exchange to meet CalAIM’s initiative for Data Exchange Framework (DxF) for improved integration of health and social services. The vendor helps counties meet these requirements by facilitating data exchange across health care providers, public health departments, and social services, ensuring compliance with state mandates. Their services also enable seamless data exchange with hospitals, clinics, labs, and other health entities, ensuring access to comprehensive data in real time. This collaboration allows for effective allocation of resources, achievement of coordinated care across different sectors, and improved health equity. This agreement allows the Department to fulfill State mandates and achieve goals for improving community health.
