a) AGREEMENT - Request approval and authorization for the Executive Director to sign the agreement with Data Strategy Consulting, LLC for the provision of consulting services in the development of the Public Health surveillance infrastructure and Community Health Assessment (CHA) update, with the maximum compensation amount not to exceed $120,000, effective 2/1/26 and shall terminate 10/31/27

Financial Impact:
The funding sources for this Agreement are the CHA/CHIP Grant, and the California Strengthening Public Health Initiative (CASPHI). Funds are budgeted in accounts 2024, and 2011, sub RU 800.
Background Information:
The Contractor will assist TCHSA Public Health in establishing a community health surveillance infrastructure system, as well as assist Public Health’s accreditation goals by supporting changes needed to the Community Health Assessment (CHA). There are five (5) key activities the Contractor will perform to support these goals. The activities are:
1. Implementation of the Tehama County Public Health Surveillance Plan.
2. Integration of selected CHA indicators, Community Health Improvement Plan (CHIP objectives, and key surveillance plan data in the VMSG Performance Management System.
3. Assisting TCHSA Public Health in producing external data information.
4. Assisting in activities associated with updating the 2023 Community Health Assessment (CHA).
5. Respond to data requests and provide technical assistance to the Epidemiologist, as requested.
