a) AGREEMENT - Request approval and authorization for the Interim Executive Director to sign the Agreement with Hanes Flooring Inc. for the purpose of repairing and replacing flooring at the Tehama County Medical Clinic, with the maximum compensation not to exceed $69,870, effective upon signing and shall terminate upon project completion (Subject to receipt of required insurance documentation)

Financial Impact:
Costs associated with this Agreement will be paid for from the Partnership HealthPlan of CA Access Grant. No General Fund dollars will be used.
Background Information:
This Agreement will allow for much-needed flooring repairs and renovations to be done in the east wing of the Tehama County Medical Clinic with the goal to expand services offered to clients at the Clinic.
