Financial Impact:
Partnership HealthPlan of California will reimburse Tehama County Community Action Agency for approved Enhanced Care Management services provided to members of Partnership HealthPlan, at a per-member, per-month rate. This reimbursement amount will depend on the services provided and the duration of the services. There is no impact to the General Fund.
Background Information:
This new agreement will allow Tehama County Community Action Agency (TCCAA) to provide specific and tailored Enhanced Care Management services to members of Partnership HealthPlan during the term of the contract. Enhanced care management services include client specific services that assist clients with clinical and non-clinical needs. With approval of this agreement, TCCAA will submit claims for the provision of services to Partnership HealthPlan.
