Background Information:

California Advancing and Innovating Medi-Cal (CalAIM) is a statewide initiative administered by the California Department of Health Care Services (DHCS) to improve coordination of health care and supportive services for Medi-Cal members. A key component is the Justice-Involved (JI) Reentry Initiative, which supports eligible individuals during the 90 days prior to release from incarceration and their transition back into the community.

On January 26, 2023, DHCS received approval under the CalAIM 1115 Demonstration for Providing Access and Transforming Health (PATH) capacity building funds to support the planning and implementation of pre-release and reentry services, with authorization to distribute up to $410 million statewide. Tehama County’s implementation requires coordination amongst the Jail, Health Services, Social Services, behavioral health providers, Medi-Cal managed care plans, and community-based organizations to facilitate access to health care, behavioral health services, medications, and other supportive services prior to release and following release into the community.
