Agree #2023-90) for the purpose of planning and systems development for a vaccine equity program, thereby extending the contract termination date at no cost, effective 1/1/21 and shall terminate 6/30/26

Financial Impact:
This a “no-cost” extension. The maximum amount for this agreement is $130,000. The funds for this agreement are budgeted in the Public Health ELC Expanded Fund.
Background Information:
The Board of Supervisors approved the original agreement on 10/19/21, for the contract term of 1/1/21 - 6/30/2023, and for the maximum compensation amount of $70,000. On 4/18/23, the Board of Supervisors approved Amendment #1, that extended the contract term to 6/30/24, and increased the maximum compensation amount to $130,000. Amendment #2 will extend the contract term to 6/30/24.
United Way of Northern California is a non-profit organization that aims to serve at risk populations which have been identified based on various vulnerabilities including ethnic groups, social determinants of health, and barriers to accessing care or who are homeless or at risk of homelessness. Populations such as, LatinX, LGBTQ, Native and Indigenous, Undocumented Community Workers, Agricultural Workers, and those who are deaf and hard of hearing or who have low visual acuity.
